Please have the Broker send rate sheets to your Logistics Coordinator directly (except for after hours).

Please complete all areas of the form, and your Coordinator, will be notified.  Once we receive the rate sheet from the Broker,  your Coordinator will email you a copy of the rate sheet and send your final dispatch email.
